Employees will be asked to complete an OSPI form (available at BIPD), provide photo identification, and pay the fingerprinting fee. For more information and to schedule an appointment, please call BIPD at 206.842.5211.

Not available to complete fingerprinting locally? Please visit the OSPI site to review other acceptable fingerprint service providers.
